Jersey City Medical Center has lost a computer disk that contained Medicaid patient information, including Social Security numbers.
The disc was sent to a company used by the state to review payments and United Parcel Service failed to deliver it on June 16.
A hospital spokesman tells The Star-Ledger of Newark Medicaid patients who were treated at the hospital in 2011 are advised to monitor their credit reports and financial accounts. The hospital has offered the patients a year's free membership in a identity-theft monitoring service.
The disk did not contain addresses or details and treatments.
