State authorities have revoked the license for a west Philadelphia funeral home and levied the maximum $100,000 civil penalty.
Officials said the Pennsylvania State Board of Funeral Directors on May 12 imposed a penalty of $90,000 against Blair Hawkins for violations in services to three deceased individuals and imposed a $10,000 penalty against Hawkins Funeral Services Inc. as an unlicensed funeral establishment.
The Pennsylvania Department of State said the board found that Hawkins "engaged in gross incompetency, negligence or misconduct.''
Officials said inspectors last year found three deceased individuals in the funeral home, two of them in a state of decomposition "with bags of trash placed atop and around them.''
Defense attorney John McMahon Jr. didn't immediately return a call seeking comment.
